As indicated in the Student Conduct Code, any member of the university community may report an alleged violation of the Student Conduct Code within 90 days of an alleged violation. To report a potential violation of the Student Conduct Code- including Academic Misconduct, submit a report including the following information:

1. Name of student
2. Date and time of the incident(s)
3. Description of the incident(s)
4. Name(s) of witness(es), including yourself
