Refer a Student
As indicated in the Student Conduct Code, any member of the university community may report an alleged violation of the Student Conduct Code within 90 days of an alleged violation. To report a potential violation of the Student Conduct Code- including Academic Misconduct, submit a report including the following information:
- Name of student
- Date and time of the incident(s)
- Description of the incident(s)
- Name(s) of witness(es), including yourself
Please be advised that you may be called as a witness for the student’s conduct hearing. As a witness, you will be asked to give a statement regarding the incident that occurred. If you are unable to attend the scheduled conduct hearing, you may submit a witness statement via e-mail, or come to our office prior to the conduct hearing.
